Maximizing exposure therapy: insights into working mechanisms of exposure therapy

The therapeutic strategy of repeated exposure to fear producing stimuli is effective for fears and anxiety disorders, but a substantial number of individuals fail to respond. Translation from the basic science of fear extinction and inhibitory regulation offers strategies for increasing response rates to exposure therapy. The underlying theories and evidence for these strategies will be presented, including: experiential violation of expectancies; multiple feared stimuli; occasional aversive experiences;  variable stimuli; retrieval cues and mental reinstatement; positive valence training; and affect labeling.
